Rant after a long day

Sometimes I come across patients who clearly aren't fluent in English and are slightly educated but insist on speaking in English. I can speak in a couple of the local languages and every time I try to, they respond in English. What they say makes almost no sense. Please, I can understand the local language, please tell me what your problem is so that I can help you
